HELLO CHOIR! :)

Here's a to-do list for Riva, be sure to take a look at it! Take note of the deadlines, but it's best to get everything done ASAP.

• Submit FULL payment by 3 March 2010, otherwise your place will be forfeited.
• Submit official consent form together with your passport photo by 3 March 2010.
• Ensure that your passport has at least 6 months validity. If not, make a new one by 3 March 2010.
• Submit a photocopy of your passport details page by 3 March 2010.
• Get the H1N1 vaccine before 12 March 2010.
• Submit a photocopy of the H1N1 vaccination certificate before 12 March 2010.

For those applying for the Opportunity Fund, the following additional documents are required by 3 March 2010:

• Photocopies of the Identity Card (both sides) of every person in the immediate family.
• Photocopies of the Student Pass (both sides) of every person in the immediate family who is schooling.
• Latest payslip (i.e. January or February 2010) and latest income tax declaration (i.e. 2009 / 2010).
• [If applicable] Signed declaration of unemployment.

On a side note, any seniors willing to donate/loan their gowns/blazers/ties to us? We'd really appreciate it! You can drop them off with us at the AVA room anytime during our practices between Tuesday and Friday. Or you can just stuff the item(s) into Ms Hua's pigeon hole. It'd also be great if you could leave your name and contact number with the stuff, so that we can contact you. Thank you again in advance!
